A lender was paying analysts to re-key figures out of PDFs. We built a document pipeline with extraction, confidence scoring and a review queue that routes only the uncertain cases to a human.

Problem: Six-day turnaround, error-prone manual entry.
Built: Extraction service, eval suite, reviewer tooling.
Outcome: Same-day decisions on 71% of applications.

Peak-hour load was taking the planning tool down weekly. We rebuilt the scheduling core around an event log, added real observability, and cut the query surface by two thirds.

Problem: Weekly outages at peak, no visibility into why.
Built: Event-sourced core, tracing, load harness.
Outcome: p99 from 4.2s to 380ms; no peak outage since.

Retrieval over a decade of protocol documents, with citations back to the source paragraph and an evaluation set written by the clinicians themselves — so quality is argued with data, not vibes.

Problem: Answers nobody would act on without checking.
Built: Hybrid retrieval, citation UI, clinician eval set.
Outcome: Answer acceptance up from 38% to 86%.
